Do you have an ad set up to display in 2 separate zones on the same page but it only displays in one?
If so is there any chance the ad is in the same campaign, but targeted to two different Zones?
By default, our adserver prevents the same campaign from being displayed twice on a page.
You can work around this by putting the two placements in separate campaigns.
